To evaluate a referee, please complete the form below. Please remember, you do not need to identify who the referee was and the referee is not required to supply their name when asked. By completing the form the Referees Branch can determine who the referee was based on the match information provided.
Alternatively, you can download the form and submit it via mail, email, fax or by dropping it into the ESFA office. The form will be submitted to the Referees Branch, via the Referees Coordinator.
Referee Evaluation Form
The form is anonymous and supplying your name is optional. Information about club and match details are necessary to identify the referee of your match and are used for this purpose.
Please do NOT use this form if you did not have an ESFARB referee.
